Diagnosis With Brief Discussion

Chief Complaint
colon cancer w/u
Reviewy
Pulmonary MALT lymphoma
- Low-grade marginal zone B-cell lymphoma of MALT
- m/c primary pulmonary lymphoma (90%)

Pathogenesis
- Not known clearly, but
- Chronic inflammation of the BALT
- By various antigenic stimuli: smoking, chronic mycobacterial infection, autoimmune disease (Sjogren's syndrome, SLE, RA)
- Malignant monoclonal lymphoid proliferation

Imaging findings: various
- Multiple, bilateral / Peribronchovascular distribution
- Nodule, mass, mass-like consolidation (= alveolar space filling)
- with air bronchogram
- with or without bronchial dilatation
- Micronodules / Bronchiolitis (~20%)
- GGO (~25%)
= lymphomatous infiltration of interlobular septa and alveolar wall

Less frequent findings
- Septal line (~10%)
= infiltration along bronchovascular bundle and interlobular septa
- Mediastinal/Hilar lymphadenopathy (~15%)
- Mild pleural effusion (~10%)
